The file also includes information that is necessary for the treatment and that, after explicit written permission from the client, I have requested from another healthcare provider, such as the general practitioner.

The therapist at Therapie Amsterdam Noord does her utmost to guarantee the client’s privacy. This means that the client’s personal and medical data is handled very carefully. In addition, it is ensured that unauthorized persons do not have access to this data. Only the therapist at Therapie Amsterdam Noord has access to the data in the client’s file. In addition, the therapist at Therapie Amsterdam Noord adheres to the legal obligation of confidentiality, or professional secrecy.
The data from the file can be used for the following purposes:
To inform other healthcare providers, for example when therapy has been completed or when a referral is made to another practitioner. This only happens with the client’s explicit written permission.
For anonymized use during intervision meetings, which are imposed by the professional association NBVH.
Personal data from the file are used for financial administration, so that an invoice can be drawn up.

If it is necessary to use the client’s data for other reasons, this will first be discussed and will only be done after explicit written permission.

Retention obligation
Under the WGBO, the file is kept for 20 years after the end of the treatment agreement. The period may be longer if this is necessary in view of the treatment (for example if the client has a chronic illness).
The client has the right to inspect, change, delete and receive the file. There is no right of access for surviving relatives.

Privacy on the invoice
The invoice contains the information requested by the health insurer, so that the client can declare this invoice to his/her health insurer. The client details stated on it are:
Name, address and place of residence
Date of birth
Date of treatment
A brief description of the treatment, such as: “Consult or intake”
The costs of the consultation

Minors
According to the patient rights in the WGBO, competent minors between the ages of 12 and 16 have authority over themselves and their parent(s). Parent(s), caregiver(s) or legal representatives of minors up to the age of 16 have the right to co-decide on the treatment. Parent(s), caregiver(s) or legal representative have the right to information and access to the file, when this is linked to the right to co-decision for the treatment. There is an exception to this right of access, namely when the therapist at Therapie Amsterdam Noord is of the opinion that the exercise of certain patient rights is against the interests of the client. Competent clients aged 12 and older are themselves authorized to give permission for confidentiality to be breached.
Personal data of minors (clients under the age of 16) will only be processed if written permission has been given by the
the parent(s), guardian(s) or legal representative(s).

Reporting code
Therapie Amsterdam Noord uses the Domestic Violence and (Child) Abuse Reporting Code, as has been legally required for healthcare providers since July 1, 2013.
